A  Study on  the Role  of Pharmacy  and  Pharmacists  in Community  Care

Abstract

Introduction  The  aging  population  in  Korea  is  already  serious,  and  by  2026,  it  is 

expected  to  enter  a  super-aged  society  where  the  elderly  account  for  20  percent  of  the 

population.  As  the  demand  for  care  has  soared,  it  has  become  a  universal  problem  for 

the  majority  of  the  people,  and  the  Ministry  of  Health  and  Welfare  is  implementing 

community  care  initiatives.  We  will  look  at  the  role  of  pharmacies  and  pharmacists  in 

the  community  care  system.  It  looks  at  the  institutional  basis  needed  to  diversify  the 

pharmacist's  field  of  activity  and  contribute  to  public  health  care  in  the  community,  as 

well  as  foreign  cases  such  as  Medication  Therapy  Management(MTM)  and  Home 

Medicines  Review(HMR).

Purpose  This  is  to  examine  the  role  and  importance  of  pharmacies  and  pharmacists  in 

Korean  community  care.  And  consider  how  to  expand  the  pharmacist's  field  of  activity.

Korea  is  now  moving  toward  a  super-aged  society,  so  the  Ministry  of  Health  and 

Welfare  has  implemented  a  pilot  community  care  project.  In  this  project,  pharmacies 

and  pharmacists  are  implementing  Visiting  Pharmacist  Service.  The  purpose  is  to  think 

about  what  efforts  are  needed  to  establish  such  medical  services  within  the  system  and 

to  look  at  the  successful  foreign  medical  services  that  have  been  preceded.

Method  Seeing  pharmacies  that  co-exist  with  local  residents  and  have  close  relationships 

with  local  communities,  I  expanded  my  thoughts  and  thought  about  the  pharmaceutical 

service.  And  we  searched  local  journals  and  papers  to  investigate  how  the 

pharmaceutical  service  are  currently  linked  in  the  community  care  business  and  what 

should  be  supplemented.  The  main  project  provided  by  pharmacists  in  Korea's 

community  business  is  'Visiting  Pharmacist  Service'. 

We  looked  at  the  current  and  future  medical  services,  the  actual  medical  services 

implemented  in  Korea  and  their  achievements,  and  the  overall  institutionalized  and 

settled  medical  services  in  foreign  countries,  and  how  they  should  be  applied  to  Korea, 

and  referred  to  and  reviewed  visiting  medical  fees.

Result  Most  developed  countries  have  already  entered  an  aged  society  in  the  early  20th 

century.  It  has  established  a  health  care  system  that  is  appropriate  throughout  society. 

Korea  is  different  from  developed  countries.  In  particular,  the  decline  in  the  ratio  of 

economic  population  becomes  a  serious  social  problem.  And  it  will  pose  a  serious 

threat  to  the  national  health  insurance  finances. 

Currently,  the  National  Health  Insurance  Corporation  and  the  Ministry  of  Health  and 

Welfare  are  making  many  policy  preparations  and  projects  for  community  care  projects. 

Doctors,  pharmacists,  and  nurses  visit  the  patient's  residence  to  check  his  or  her  health. And  preventing  misuse  of  drugs.  This  not  only  helps  promote  national  health  but  also 

can  lead  to  stabilization  of  national  health  insurance  finances.  It  is  the  common  view  of 

many  health  and  medical  experts  that  the  paradigm  should  be  shifted  from 

hospital-centered  treatment  to  regional-centered  prevention  and  care.

1)  In  the  United  States,  there  are  many  policies  for  drug  management  for  the  elderly. 

Mediation  therapy  management  (MTM)  is  a  service  provided  directly  to  patients  by 

pharmacists.  It  is  a  medicinal  service  that  covers  the  overall  course  of  drug  use, 

including  evaluation  of  health  condition  and  medication  course,  medical  history  review, 

drug  treatment  plan  establishment,  monitoring  patient  response,  identification  and 

resolution  of  drug-related  problems,  education  and  training  for  improving  drug 

compliance  and  proper  drug  use.  For  Medicare  subscribers,  one  of  the  U.S.  social 

security  systems,  drug  treatment  effects  are  being  enhanced  through  proper  use  of 

drugs  and  management  of  side  effects.

2)  The  elderly  pharmacist  system  is  part  of  the  professional  pharmacist  system. 

The  professional  pharmacist  system  began  in  1974  in  the  United  States,  and  Korea  has 

been  implementing  it  for  hospital  pharmacists  since  2010.  In  order  for  Visiting 

Pharmacist  Service  projects  to  be  professionally  provided  to  a  large  number  of  senior 

citizens,  specialized  institutions  and  personnel  in  this  field  are  needed.

Currently,  Korea's  professional  pharmacist  system  is  applied  only  to  hospital  pharmacists 

and  is  concentrated  in  the  Seoul  metropolitan  area,  so  it  needs  to  be  expanded 

nationwide.  It  is  necessary  to  legislate  the  system  by  extending  it  to  local  pharmacies. 

And  if  it  can  be  linked  to  the  visiting  drug  business  to  suit  the  situation  in  Korea,  the 

safety  of  elderly  people's  drug  use  could  be  further  enhanced.

②  Visiting  Pharmacist  Service  in  Australia

In  Australia,  a  team  of  pharmacists  and  general  prescriptions  implemented  the  Home 

Medicines  Review  (HMR).

③  Examples  of  Visiting  Pharmacist  Service  conducted  in  Korea  and  what  needs  to  be 

supplemented  Chronic  diseases  such  as  high  blood  pressure  and  diabetes  are  increasing 

with  the  aging  population,  and  account  for  a  large  portion  of  the  burden  of  disease. 

Korea's  Visiting  Pharmacist  Service,  which  took  place  from  July  to  mid-December  2018, 

was  successful.  A  home  visit  was  made  to  identify  general  medicines,  health  foods,  etc., 

not  limited  to  prescription  drugs.  And  the  pharmacist  offered  customized  services. 

Through  this  project,  there  was  an  effect  of  improving  duplicate  medication.  And  drug 

compliance  increased.  However,  there  is  a  possibility  that  information  on  drug-related 

problems  could  not  be  delivered  to  the  prescription  doctor  at  the  next  visit  to  the 

medical  institution.  Therefore,  a  more  standardized  information  delivery  system  needs  to 

be  established.Conclusion  Pharmaceutical  services  are  essential  to  the  promotion  of  national  health.

Prevent,  identify,  and  resolve  drug-related  problems  to  promote  drug  treatment.  And 

ultimately  improve  the  patient's  quality  of  life.  The  advancement  of  medical  services  will 

save  health  insurance  finances.  Local  pharmacies  and  pharmacists  are  a  significant  part 

of  the  entire  health  care  supply  system.  Therefore,  in-depth  discussions  on  the  functions 

and  roles  that  local  pharmacies  and  pharmacists  are  responsible  for  in-depth  community 

care  projects  are  needed.  The  government  should  expand  pilot  projects  for  visiting 

medical  services  that  have  been  implemented  so  far.  And  demonstrate  the  positive 

results  of  the  pilot  project,  and  present  a  pharmacist  service  standard  model  that 

reflects  the  value  of  the  pharmacist  service.  Furthermore,  economic  assessment  and 

policy  development  are  needed  to  institutionalize  medical  services  and  determine  their 

prices.  In  order  to  diversify  the  areas  of  pharmacists'  activities,  specialized  personnel 

should  be  trained  through  the  readjustment  of  the  education  system.  The  institutional 

foundation  should  be  established  to  provide  more  professional  medical  services  and 

contribute  to  public  health  care  in  the  local  community.  It  is  also  necessary  to  establish 

a  cooperative  system  between  health  care  and  medical  functions  so  that  drug-related 

issues  can  be  shared  smoothly  with  each  other.  We  should  refer  to  successful  foreign 

medical  service  cases  and  institutionalize  them  to  suit  Korea's  situation.
